The Stepping Strong Transforming Recovery After Injury for the Long-term (TRAIL) Clinic, originally launched as a Stepping Strong Innovator Award project, is now a fully operational care model at Brigham and Women’s Hospital where it provides vital support to patients in their recovery from traumatic injuries.

A New Approach to Care

As New England’s first collaborative care model for traumatic injury survivors, the Stepping Strong TRAIL Clinic provides a centralized solution for patients requiring a wide range of specialized care from various disciplines. Here, patients receive integrated, coordinated, and compassionate treatment that not only addresses their medical needs but also helps overcome logistical challenges that may otherwise hinder their recovery.

Patient Information

The clinic is not currently open to external patients. All patients must be identified and referred by an inpatient provider. Please see the “Referrals” tab for more information.

  • Admitted to the Brigham general surgery trauma service for traumatic injury
  • Must be 18 years of age or older
  • Increased risk for poor long-term outcomes
    • Injuries resulting from interpersonal violence
    • Limited English proficiency
    • Weak social support network
    • Low perceived socioeconomic status
    • Substance Use Disorder

During the pilot phase, eligible patients will be identified by inpatient staff at the Brigham and Women’s Hospital. The Stepping Strong TRAIL Clinic is not currently open to external referrals.
